Assistant Manager/Manager, Projects Office – Disease Management

Responsibilities:

The Assistant Manager/Manager is responsible for the development and reviews of disease management programme(s), manage all the operational and administrative aspects, with advanced responsibilities in establishing programme objectives, deliverables, goals and monitor performance against projections, key performance indicators and international benchmarks.

The post holder will work closely with the clinical teams to manage population health through the optimal utilization of limited healthcare resources to provide patients with evidence-based, cost-effective and quality healthcare.

MAIN DUTIES AND RESPONSIBILITIES:

Clinical Programme Management

- Funding/Grant Applications.

- Budget Management & Cost Accounting

- Manpower Capacity Management.

- Protocols, Standard Operating Procedures (SOP), and Service Standards.

- Patient Assessments and Records.

- Assists to draft and develop patient education materials. Also, coordinates the production of patient education materials.

- Plans annual community education and outreach initiatives. Organises and coordinates annual public forums, workshops, exhibition, etc.

- Date Management & Analysis.

- Progress Reports.

- Establishes collaborative partnerships with a range of stakeholders and manages stakeholders’ expectations.

- Acts as key liaison person between MOH and NHG on all matters concerning the Programmes.

- Secretariat Support to the Heart Failure Disease Management Workgroup.

- Continuous Quality Improvement Initiatives.

Provides secretariat support to other cluster-wide working committees.

Participates in cluster-wide working committee and in departmental level meetings.

Performs other duties as directed by Supervisor and Senior Director, Clinical Programmes, NHG HQ.

Requirements:

· Degree or advance diploma in nursing or allied health disciplines

· Must have prior clinical experience in nursing, medicine or allied health disciplines that commensurate with the job

· At least 8 years of working experience in related patient care areas

· Possess strong administrative, budget management and project coordination skills

· Some knowledge/training in financial management, health information management and utilisation management would be useful

· Must possess working knowledge of case management and healthcare utilization principles

· Able to provide clinical leadership and direction for case management and disease management

· Proficient in various computer applications, including spreadsheets

· Independent, self-motivated and result-oriented

· Excellent communication and coordination skills

· Good analytical skills with the ability to see the “big picture” while still maintaining appropriate attention to details

· Good critical thinking skills
