Case Manager, Projects Office-Disease Management

Responsibilities:

You will be responsible to case manage patients with chronic diseases under NHG’s Disease Management Programmes. You will educate patients on the disease progress, encourage self-management, identify at-risk patients, monitor their progress through telephones and home visits (when necessary) and review patients’ care plans at both inpatient and outpatient settings. You will also perform some outcome management and tracking of data.

Requirements:

- Candidates with a recognized degree in Nursing with 3 – 5 years in clinical or general patient management are preferred

- Experience in case management, patient education, data management and utilization review will be an advantage

- Additional experience with cross-functional / multi-disciplinary teams will be beneficial.

- Proficiency in MS Office applications is required and basic statistical knowledge will be an advantage.

- You are self-motivated, independent and results-oriented, and possess good communication skills.
